Social Prescribers connect people with local community activities and services that can help improve their health and wellbeing. They also offer support with a wide range of non-medical issues. Our team has a particular interest in working with asylum seekers, individuals with long-term health conditions affected by housing or fuel poverty, and people with learning disabilities. We also support carers, those affected by dementia, armed forces veterans, and individuals experiencing social isolation. In addition, we can assist with housing and benefits, as well as helping people get involved in volunteering opportunities.
