Blood Pressure Pathway at Medics Primary Care Network
At Medics PCN, we offer a dedicated service to help diagnose and manage high blood pressure (hypertension). If you’re a new patient or have recently had a high blood pressure reading, we will guide you through a clear and supportive pathway.
What to Expect
1. Initial Assessment
If your blood pressure reading is raised, we’ll start with further checks to understand whether you might have hypertension.
2. 24-Hour Blood Pressure Monitoring
You may be offered an ambulatory blood pressure monitor. This device checks your blood pressure regularly over 24 hours, helping us get a more accurate picture.
3. Diagnosis and Next Steps
Once we review your results, we’ll confirm whether you have high blood pressure. If needed, we’ll offer further tests like an ECG (heart tracing) to check your heart health, blood tests and a urine test.
4. Support and Management
If you are diagnosed with hypertension, we’ll work with you to create a personalised care plan. This may include lifestyle advice, regular monitoring, and medication if appropriate.

Who Is Most at Risk?
Research shows that if you are from Indian or Black ethnic backgrounds, you are particularly at higher risk of developing high blood pressure and its related complications. These groups are a key focus for the ICB IHI Hypertension Learning Network, and we are committed to ensuring inclusive and targeted care.
